Educational use only. Free and paid reference data and tools are for educational and operational support. They are not a substitute for official CMS, AMA, payer, or legal/coding advice. Always confirm current requirements with the authoritative source.

1. Acceptance

By accessing or using NexLookup (the “Service”), you agree to these Terms. If you do not agree, do not use the Service.

2. Accounts

You are responsible for maintaining the confidentiality of your account credentials and for all activity under your account. Provide accurate registration information. You must be at least 18 years old to create an account.

3. Acceptable use

  • Do not upload protected health information (PHI), patient charts, or claim files containing patient identifiers.
  • Do not attempt to reverse-engineer, overload, or disrupt the Service.
  • Do not use the Service for unlawful purposes or to violate third-party rights.
  • Automated scraping of paid tools beyond reasonable use is prohibited.

4. Free tools and paid subscription

Public reference tools (ICD-10-CM, CARC/RARC, single NPI, HCPCS, modifiers, POS, taxonomy, educational calculators, and /reference pages) are free and ungated.

Agency workbenches are available under a single paid product: NexLookup Agency Suite (monthly or annual). Pricing is shown on the product/catalog pages and may be updated by the operator. Admin-granted trials or starter access may be provided at our discretion.

5. Billing, cancellation, and refunds

Subscriptions renew according to the selected cycle until canceled. You may cancel through your account or by contacting support; access continues through the end of the paid period. Refund policy is determined by the site operator and payment processor terms (Stripe / Razorpay). Chargebacks for legitimate paid service may result in account suspension.

6. Intellectual property and data

Application code is available under the MIT License (see License). CMS, NUCC, and other public code sets used for reference are public-domain or publicly published reference data and are not proprietary to NexLookup. AMA CPT long-form descriptions are not reproduced.

7. Disclaimer of warranties

THE SERVICE IS PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. We do not warrant that code descriptions, SLA calculations, or workbench data are complete, current, or error-free. Always verify against official CMS, payer, and professional sources.

8. Limitation of liability

To the maximum extent permitted by law, NexLookup and its operators shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or for lost profits, revenue, or data, arising from use of the Service. Aggregate liability shall not exceed the amount you paid us in the twelve (12) months preceding the claim.

10. Changes
